An official/unofficial transcript of a student’s academic record at MacCormac College is available only through the Registrar’s Office upon a written request of the student. Transcripts are mailed within three to five business days from the day the request is received in the Registrar’s Office. We honor requests as quickly as possible however; there may be some delay during busy periods such as registration, final exams, commencement, etc. No fax requests accepted.
The College reserves the right to withhold transcripts of persons who have past due monetary obligations such as tuition, fees or materials. Each student is entitled to one free transcript following graduation. There is a charge of $5.00 for each official/unofficial copy.
Official Transcript- An official transcript bears the MacCormac College Seal and a signature certifying its authenticity. Unofficial Transcript- Is the student’s copy.
Transcript Fee- There is a charge of $5.00 for each official/unofficial copy. Cash, check, and money orders are accepted.
Transcript Requests by mail
Send a letter requesting your transcript to:
Attention: Transcripts/Registrar’s Office, MacCormac College, 29 E. Madison, Chicago, IL 60602.
Please include: your name (name used when attending or maiden name), current address and phone number, Social Security number, dates of attendance, indicate if you need it official or unofficial, your signature, and the name and address of the institution or agency where your transcript should be sent. Enclose a $5 check or money order payable to MacCormac College. An official/unofficial transcript requested for mailing will be mailed within three to five business days from the day the request is received, except during peak periods when the process may take a little longer.
Requests in Person
In person, visit the Registrar’s Office located on the 2nd floor in room 203. There, you can fill- out the Transcript Request Form and pay the transcript fee of $5.00.
Transcripts are not given the same day of request.

Employers must fax a signed release form from the student before any information can be given.
Students requesting a letter verifying enrollment status for a job, insurance, etc. must submit a written request to the Registrar’s Office. Once completed enrollment letters can be mailed or picked-up by student in the Registrar’s Office.
***We regret that phone or electronic mail requests are NOT acceptable.
College policy strictly limits the release of student information. For this reason, not all services are available via electronic mail. Some services, such as transcript request or enrollment verifications, require a signature for release.
